(1) Fix def_exc relevant constant names.
(2) Use DEF_EXC instead of ATT_ISR for MPU exception handlers in
exphdrs.c.
(3) Intergrate common processing in exception handling in IA32 in hosexp.h
and pacent.S.
(4) Refine titile comments.
(5) Fix spell-mis in exphdrs.c
